It's that time again! Time to start preparing for the 9th Annual Kansas Youth Leadership Forum. This year's forum will be held on June 16-20, 2009 at Washburn University in Topeka. If you are interested in participating as a delegate or volunteer, please fill out the corresponding application below.

Brief Information:
- Students must complete ALL information on pages 1 through 4 of this application.
- Please type or print with black ink.
- Mail the application to the address below.
- Please see Fact Sheet for additional application instructions.
To be eligible for the KS Youth Leadership Forum, students must:
- Reside in Kansas
- Have a disability (as defined by the Americans With Disablities Act)
- Be in the 11th or 12th grade as of December 31 of the current year
- Have demonstrated leadership potential in school and the community
Things to complete for your application packet:
- Application Form (4 pages)
- Two completed reference forms
- Written or taped response to two topics
